Iowa Law requires that students entering Kindergarten and 9th grade must submit an Iowa dental screening certificate. This is to be submitted to the school no later than four months after the first day of the school year. This form is available on the School website and at the school offices. Kindergarten screening must be done any time after age 3. Ninth grade screening must be no older than one year at the start of the school year. Dental Screening Form
